Who We Support
International mental health repatriation can involve much more than arranging a flight. The patient may need clinical review, careful preparation, a suitable escort, liaison with hospitals or authorities, and a clear plan for the full journey.
SkyCare supports hospitals, mental health professionals, insurers, assistance companies, embassies, families and individuals who need to move a patient internationally for further care, return home or transfer between facilities.
Every case is assessed individually. We consider the patient’s current presentation, physical health, communication needs, medication, known triggers, capacity and consent where relevant, journey length, destination and the level of support needed throughout the transfer.
Mental Health Repatriation Transport Options
The safest transport method depends on the patient’s clinical needs, journey and individual risk assessment. SkyCare can coordinate dedicated air ambulance, commercial medical escort and specialist ground transport, with the least restrictive suitable option preferred wherever possible.
Dedicated Air Ambulance
For patients who are not suitable for scheduled commercial travel, a dedicated air ambulance can provide greater privacy, control of the environment and direct routing. The clinical team and transport plan are selected around the individual patient’s needs.
Commercial Medical Escort
Where a patient is stable and suitable for scheduled air travel, a specialist medical escort may be appropriate. We coordinate airline medical clearance, airport arrangements and the clinical support required throughout the journey.
Ground & Secure Mental Health Transport
For appropriate journeys, SkyCare can provide specialist road transport. In the UK, our discreet secure mental health ambulance gives the clinical team an additional option where assessment indicates that a higher-support transport environment is required.

SkyCare approaches international mental health transport using UK principles of dignity, individual assessment, proportionality and the least restrictive approach appropriate to the patient's needs.
Our aim is to reduce distress through careful preparation, appropriate staffing, clear communication, reassurance, de-escalation and contingency planning. The transport plan is built around the individual rather than simply around the aircraft, vehicle or timetable.
We work with the referring and receiving clinical teams and consider the legal and clinical framework that applies at each stage of the journey. International cases can involve different laws and authorities, so we do not assume that one country's legal status automatically applies in another.
Clinical Planning Before Travel
Good mental health repatriation starts before the patient leaves the ward, hotel, home or treatment facility.
- Clinical review: current mental and physical health, medication, recent behaviour and relevant history.
- Risk assessment: likely triggers, distress, absconding risk, self-harm risk, aggression risk and practical travel concerns where relevant.
- Travel planning: route, timing, airport environment, seating, transfers and contingency arrangements.
- Receiving arrangements: confirmation of the destination, handover requirements and the team accepting the patient.
- Communication: clear information for the patient, family and professionals involved, wherever appropriate.
Specialist Secure Mental Health Transport in the UK
Some patients need a more controlled ground-transport environment than a standard ambulance can provide. SkyCare operates a dedicated discreet secure mental health ambulance in the UK, giving our clinical team an additional option where an individual risk assessment indicates that a higher-support transport environment is appropriate.
The vehicle is part of a wider clinical transport plan rather than a substitute for assessment, de-escalation or appropriate staffing. Where a standard ambulance or less restrictive option is suitable, that remains the preferred approach.
This capability can be particularly useful for international repatriation cases where the patient arrives in the UK and requires a carefully planned onward transfer to an appropriate hospital or facility.
