Commercial Medical Escort Flights for Safe International Repatriation

SkyCare provides commercial medical escort flights for medically stable patients who need professional clinical support while travelling on a scheduled airline. A qualified British nurse, critical care paramedic or doctor can accompany the patient throughout the journey, while our coordination team manages the medical and travel arrangements required to bring them home safely.

We arrange medical repatriation on commercial flights worldwide, including hospital liaison, review of medical information, airline MEDIF and medical clearance, appropriate seating, wheelchair and airport assistance, approved in-flight oxygen where required, medical equipment, medication planning and onward ground ambulance transport.

For suitable patients, a medically escorted commercial flight can provide a discreet and considerably more cost-effective alternative to a private air ambulance while still delivering professional medical supervision from departure to destination.

Need a Commercial Medical Escort Quote?

Tell us where the patient is, where they need to travel to and what medical support they require. SkyCare can review the case, advise whether a commercial medical escort is suitable and provide a clear quotation.

Not sure whether a commercial medical escort is appropriate? SkyCare assesses each case individually. If the patient requires a stretcher, we can arrange a commercial airline stretcher flight. Patients who are too unwell for scheduled airline travel may require a private air ambulance.

Want to understand the complete journey? See our step-by-step Commercial Medical Escort booking process.

SkyCare provides medical travel and repatriation services only. We do not provide adult companionship or private escort services.

Who Can Travel with a Medical Escort?

Commercial medical escorts are designed for patients who have been assessed as sufficiently stable to travel on a scheduled airline but need more assistance or clinical supervision than an ordinary passenger. This can include people recovering after surgery or illness, elderly or frail patients, travellers with reduced mobility or fractures, patients requiring medication or oxygen during the journey, and people returning home after hospital treatment abroad.

The appropriate escort is selected according to the patient’s condition and anticipated care requirements. Depending on the case, SkyCare may deploy an experienced flight nurse, critical care paramedic or doctor. We review the available medical information and work with the treating team and airline to establish whether commercial travel is appropriate.

What SkyCare Handles Before You Fly

A medical escort is only one part of a successful repatriation. SkyCare coordinates the journey as a complete medical transfer rather than simply placing a clinician beside the patient on an aircraft.

  • Clinical assessment and hospital liaison – obtaining and reviewing relevant medical information and communicating with treating teams.
  • Fit-to-fly and airline MEDIF coordination – supporting the airline medical-clearance process and providing the information required by its medical department.
  • Flight and seating arrangements – selecting an appropriate itinerary and seating configuration for the patient’s condition and mobility.
  • Airport assistance – coordinating wheelchairs, reduced-mobility assistance and specialist airport handling where required.
  • Oxygen and medical equipment – arranging airline-approved oxygen solutions and appropriate clinical equipment according to the patient’s needs.
  • Medication planning – ensuring necessary medication and clinical supplies are available throughout the transfer.
  • Ground ambulance transfers – arranging collection and onward transport when a bedside-to-bedside journey is required.
  • Receiving arrangements – coordinating with family, care facilities or hospitals at the destination so that responsibility for care transfers smoothly.

Our team remains involved throughout the process, giving patients and families one point of coordination rather than leaving them to manage hospitals, airlines, airports and transport providers separately.

Commercial Medical Escort vs Air Ambulance: Which Do You Need?

The right method of medical repatriation depends on the patient’s clinical stability, mobility, ability to tolerate a scheduled flight and the level of treatment required during transport.

Travel option Usually suitable for Key considerations
Seated medical escort flight Stable patients able to travel seated with medical support Requires airline medical clearance where applicable; nurse, paramedic or doctor can accompany the patient
Commercial stretcher flight Stable patients who cannot remain seated for the journey Requires airline stretcher availability, MEDIF approval and additional planning
Private air ambulance Seriously ill, unstable or higher-acuity patients Dedicated medical aircraft, intensive clinical capability and greater flexibility than scheduled airlines

There is no single solution that is right for every patient. Our team will review the case and explain the safest realistic options. Read more about Fit to Fly and airline medical clearance, or contact SkyCare for an individual assessment.

Bed-to-Bed Medical Repatriation on Commercial Airlines

Where required, SkyCare can coordinate the entire transfer from the patient’s hospital, home or care facility overseas through to their final destination. This may include an ambulance to the departure airport, assistance through the terminal, the medically escorted flight, arrival assistance and a second ambulance or suitable vehicle after landing.

This bed-to-bed medical repatriation approach is particularly valuable when relatives are trying to organise a return from another country and do not want to coordinate multiple medical, airline and transport providers themselves. SkyCare manages the operational chain so that each stage connects safely with the next.

Families, hospitals and international partners trust SkyCare to coordinate complex patient transfers with clinical oversight, discretion and compassion. Read our Commercial Medical Escort FAQs or request a quotation for your journey.

Commercial Medical Escort Services – What We Arrange

Every commercial medical repatriation is individually planned around the patient's condition, mobility and destination. SkyCare combines professional in-flight medical care with the clinical clearances and international logistics required to complete the journey safely.

Our team can coordinate airline MEDIF approval, Fit to Fly requirements, airport assistance, oxygen, medical equipment, ground ambulances and receiving arrangements. Learn about Fit to Fly and airline medical clearance, or read our medical escort FAQs.

Who Needs a Commercial Medical Escort?

A commercial medical escort is suitable for many patients who are medically stable enough to fly on a scheduled airline but require professional clinical support, mobility assistance or reassurance during the journey.

This can include elderly or frail travellers, patients recovering from surgery or hospital treatment, people with fractures or reduced mobility, and patients who require medication, monitoring or approved oxygen in flight.

SkyCare reviews each case individually and selects the appropriate nurse, critical care paramedic or doctor according to the patient's medical needs and the requirements of the airline.

We Coordinate the Entire Medical Journey

Our service goes far beyond providing a medic on the aircraft. SkyCare liaises with hospitals and treating teams, coordinates airline MEDIF and medical clearance, arranges suitable flights and seating, airport assistance, oxygen or medical equipment and ground transport where required.

Your medical escort can meet the patient at hospital, home, a care facility or the airport and remain with them through the journey. Where appropriate, we arrange a complete bed-to-bed transfer with ambulances at departure and arrival.

Patients and families have one experienced coordination team managing the medical and operational details from the first assessment until the patient reaches their destination.

Medical Escort, Stretcher Flight or Air Ambulance?

The safest transport method depends on the patient's stability, mobility and clinical requirements. Stable patients who can remain seated may travel with a medical escort on a scheduled airline. Patients who are stable but cannot sit upright may be suitable for a commercial stretcher flight.

Patients who are seriously ill, unstable or require a higher level of continuous treatment may need a private air ambulance instead.

SkyCare will review the medical information and explain the safest realistic options, allowing families and case managers to make an informed decision without having to determine the transport method themselves.

Worldwide Medical Escort & Bed-to-Bed Repatriation

SkyCare arranges commercial medical escort flights worldwide for patients returning to the UK, travelling from the UK or moving safely between international destinations.

Depending on the case, we can coordinate hospital discharge, ambulance collection, airline medical clearance, airport assistance, in-flight clinical care and onward transport after landing. This creates a continuous bed-to-bed medical repatriation pathway with clear clinical responsibility at every stage.

Our British nurses, critical care paramedics and doctors support patients with professionalism and discretion while our operations team manages the complex airline and international logistics behind the journey.

If scheduled airline travel is not clinically appropriate, we can also arrange a private air ambulance or, for suitable European journeys, specialist road ambulance repatriation.

Medical escort flight with SkyCare clinician providing in-flight care on a commercial airline

Commercial Medical Escort FAQs

A Commercial Medical Escort flight involves one of our highly trained medical professionals accompanying you on a commercial airline, providing care and support throughout your journey.

You will travel on a regular scheduled flight but with the added reassurance of having our doctor, nurse, or critical care paramedic by your side at all times.

Medically accompanied flights are a cost-effective alternative to a private air ambulance or road ambulance repatriation and are suitable for patients who are stable but require assistance during travel. This option is ideal for patients who are older or frail, recovering from surgery, have mobility issues, minor fractures, or for those who simply need medical supervision because family members are unable to travel with them.

Our team ensures you are fully supported from bedside to bedside, making your journey home as safe and comfortable as possible.

Once you instruct SkyCare to assist your family member, we will guide you through every step and take care of everything. We liaise directly with the hospitals at both ends, arrange medical clearance with the airline, and coordinate all the logistics involved in the transfer.

To help the process move as quickly and smoothly as possible, it’s important that you provide us with full contact details for the hospital where your family member is currently being cared for.

Obtaining medical clearance from the airline is often the most time-consuming part of the process, so the sooner we can start, the better.

You are under no obligation to proceed until you are fully happy to do so. We do not charge anything to begin the medical clearance process — we simply want to be ready to move quickly once you give us the go-ahead.

If the patient cannot obtain the medical clearance required for scheduled airline travel, SkyCare will review the alternatives rather than trying to force an unsuitable commercial-flight solution.

Depending on the patient’s condition and location, options may include waiting until their condition improves, arranging a private air ambulance, or using specialist ground ambulance transport where geographically appropriate.

Our team can discuss the medical information with the treating clinicians and advise which transport methods are realistically available.

Yes, absolutely.

SkyCare can fully manage stretcher flights on commercial airlines. While these flights are generally more expensive than seated medical escort flights and may take additional time to arrange, they are often the safest and only option to bring seriously ill patients home to their loved ones.

We manage the entire bed-to-bed transfer process, including all medical clearances (such as airline MEDIF approvals) and ground ambulance transfers at both departure and arrival points. The airline will arrange for the stretcher installation and privacy curtain setup on board, and our team works closely with them to ensure everything is properly in place before travel.

Our experienced team of doctors, nurses, and critical care paramedics provides continuous care throughout the journey, ensuring the patient’s dignity, comfort, and safety are maintained at every step.

Your safety and wellbeing are our highest priority.

Find out when a stretcher flight is the right choice

Yes, absolutely. We are happy to assist with arranging travel for family members or companions to accompany the patient.

However, we do advise caution. Medical escort seats and family seats are usually booked together, and because airline prices often increase between the initial enquiry and final MEDIF clearance, any additional seats booked early will also be subject to these price rises.

The more seats reserved at the outset, the greater the potential cost increase for the whole group. For this reason, we recommend limiting accompanying family members to one or two initially.

Once the patient’s seat is booked and confirmed, additional family members can then arrange their own separate bookings onto the same flight. This approach helps to keep your overall costs much lower.

MEDIF is the Medical Information Form used by many airlines when a passenger has a medical condition or requires special medical assistance. The airline’s medical department reviews the information before deciding whether the patient can travel and what arrangements are required.

SkyCare coordinates this process as part of our commercial medical escort service. We obtain the relevant medical information, liaise with the treating team and airline, and help coordinate requirements such as seating, wheelchair assistance, oxygen or medical equipment.

Airline approval can take time, so providing accurate hospital and treating-clinician contact details early can help avoid unnecessary delays. Read our guide to Fit to Fly and medical airline clearance.

It is very common for patients to require oxygen during a flight, even if they do not normally need it on the ground.

Due to changes in cabin pressure at altitude, even in a pressurised aircraft, oxygen saturation levels can decrease, which may affect the patient’s breathing.

To ensure safety, we routinely carry a Portable Oxygen Concentrator (POC) on most flights.

If a patient is already using oxygen at ground level, we will often arrange for additional oxygen support on board to maintain appropriate levels throughout the journey.

Yes, we can. SkyCare has a team of over 150 medical professionals, including doctors across all specialties, who are able to arrange hospital admissions in the UK.

To do so, we require access to the patient’s medical notes in advance to ensure the most appropriate care pathway is organised. We work with the majority of specialist hospitals in the UK, and can always find the right pathway for the patient. See our helpful advice on Private hospitals in the UK as well as Discreet transfers to London’s Clinics.

The price we quote is based on the cost of airline seats at the time of enquiry. Please be aware that delays between receiving MEDIF (Medical Information Form) approval and confirming the booking can often result in an increase in seat prices.

SkyCare cannot guarantee seat availability or the originally quoted price if there is a delay. We recommend confirming your booking as soon as possible after MEDIF approval to avoid additional charges.

Read our important notice about seat pricing and availability.

Speak to our team about the safest and most appropriate way to bring the patient home.

If you or a loved one needs professional medical support to travel after illness, surgery or injury, SkyCare can assess the case and coordinate the journey from start to finish. We can manage hospital liaison, MEDIF approval, flights, airport assistance, oxygen and medical equipment where required, as well as ground ambulance transfers for a complete bed-to-bed service.

Commercial Medical Flight from Serbia to Birmingham

My 69-year-old mum unfortunately suffered a stroke while we were flying home from Turkey, which resulted in an emergency landing in Serbia. It was one of the most frightening and stressful experiences our family has ever been through.  We didn’t have travel insurance that could help, so I spent hours contacting different medical repatriation companies and comparing quotes. SkyCare wasn’t the cheapest option, but after speaking to them, I had a feeling they genuinely cared about getting my mum home safely rather than just treating us like another customer.
